Former Students
After we receive your application, we will send you a list of documents and other items necessary to complete your admissions file.
Former C of O students must submit the following:
- C of O Transcript (obtained by the Admissions Office)
- Account Clearance (account must be clear before your application will be processed)
- Free Application for Federal Student Aid (FAFSA)
- Applicant Data Sheet
- Medical Questionnaire
- Dean's Approval (obtained by the Admissions Office)
In some cases, former students may be required to appear for a former student admission hearing. Admissions staff member will contact former student applicants to schedule the hearing if it is required.
